Please provide the following:

1) Name, Date of Birth

2) School, home, and any spring travel contact including address, phone, email

3) What is your current artistic practice and where have you studied?

4) What interests you about site specific art? (1/2 page limit)

5) Send documentation of ONE recent project (it does not have to be
about site) including up to a one-page description and a concise work sample (ie. 5 slides, 5 minutes of video, or 3 minutes of audio) and a self-addressed stamp for return of materials.

6) Will you require funding assistance to attend this program? Please include an institutional letter documenting your need (ie letter from your financial aid officer).

7) One Letter of recommendation.
